How to apply for your Wilson's Creek license plate

1. Complete the license plate form.

2. Send the license plate form and a donation of $25 or more ($50 or more for a 2-year registration) to the following address:
   
   Wilson's Creek National Battlefield Foundation
   P.O Box 8163
   Springfield, MO 65801
   

   You will receive an Emblem Use Authorization Statement, along with an application for Missouri Personalized and Special License Plates (form DOR-1716).

3. Fill out the application form (DOR-1716); submit it with the Emblem Use Authorization and the $15 special plate fee to:
   
   Missouri Department of Revenue
   P.O. Box 100
   Jefferson City, MO 65105-0100
   

   The letters and/or numbers on special plates must not exceed six (6) characters. A dash (-), apostrophe (‘), or space counts as one letter.
